Adult Plan (Ages 18-64)$80/month
Pediatric Plan (Ages 3 months-17)$50/month
Family Plan (Up to 4 members)$190/month
Married Couples Plan$110/month
College Student Plan (with valid school ID)$55/month
*Due to Medicare law, we are unable to service anyone over the age of 65 at this time.
**$80 Registration fee required at time of enrollment
Non-Member Prices
Acute Visit (colds, UTIs, minor injuries)$85
DOT Physical Exam$75
School/Sports Physical$25
Non-DOT Physical (Pre-employment, Pre-professional program, etc.)$65
*Primary care services (i.e. blood pressure management, diabetes, etc.) are exclusive to members
